This could be an AU fic.
Maybe the reason Angel the asylum manager had to leave was because Buffy stabbed him with a mop, in a fit of lovelorn passion proclaiming "I have to send you to hell!". A while later he decided it was time to leave.

That very well could have been the cause. Who were all of the vampires and demons and stuff that she killed? What about the highschool she blew up?!

The vampires and demons were the other patients. Parker, was a patient she had a brief dallience with after her heartbreak at Angel leaving the asylum.
The High School she blew up was actually the communial dining room. The residents were not best pleased.
The Giant Snake was of course, The First Matron as she has changed into many forms of evil over the years - the Master, the Mayor, Adam, Glory, Warren, The First Evil.
She gets around.

Wow. The First Maton was bad. Buffy must have gotten in a hell of a lot of trouble for all the stuff she caused. Who was Riley? A patient as well?

No, Riley was an intern. Who was soon put off of joining the healthcare profession, and had a string of affairs with other nurses and the Janitor told Buffy of this.
